Payment

We do not participate with any insurance plans, as we are listed out of network, but can provide you with a statement of service once payment is received for you to submit to your insurance company for reimbursement. We can provide you a list of our procedure codes to check the rate of reimbursement with your insurance carrier prior to scheduling.

In the event that your insurance provider sends a reimbursement check to our office, we will return the check to them and notify you so that the check is paid to you.
