Our Policies 

Client Cancellation:

For our new clients, we require at least 72-hours notice of cancellation from the initial (first) appointment scheduled. Existing clients must cancel at least 24-hours prior to their scheduled appointment. Failure to do so will result in being charged fee of the value of the session. If you have more questions regarding our cancellation policy, please call the office for more information.

Payment:

Payment is expected at time services are rendered. Upon receiving payment, a Statement of Service(s)/Superbill will be provided to you with all of the necessary coding needed for you to submit to your insurance carrier for reimbursement. The office does not submit claims on your behalf. 

Refill Request and Pharmacy Details:

Refill requests must be made at least 7-10 days prior to taking your last pill. Please include name and dose of the requested medication(s). If you plan on traveling overseas, you must provide the office with enough notice to refill any scripts you may need while you are away as well as any vacation overrides/prior authorizations. Making multiple phone calls to the office unfortunately will not expedite the process.  You must also confirm which pharmacy you would like to use to fulfill your refill request(s). The office is not responsible to confirm.

Under the laws enforced by EEOC, it is illegal to discriminate against someone (applicant or employee) because of that person's race, color, religion, sex (including gender identity, sexual orientation, and pregnancy), national origin, age (40 or older), disability or genetic information. 

The Bartky HealthCare Center does not tolerate any harassment, disrespect, or obscene language being used to any patient and/or employee. All persons shall be respectful toward one another.
